New Jersey State Law requires that children enrolled in a public school be immunized against certain vaccine-preventable diseases. These immunization requirements are specifically delineated in Chapter 14 of the New Jersey State Sanitary Code. Every school is required to maintain a record of immunizations for each child enrolled. This must include the MONTH, DAY, and YEAR for each immunization. The following requirements pertain to ALL children entering Pre-Kindergarten during the 2011-2012 school year.
DPT (Diphtheria, Pertussis, Tetanus)
A minimum of four doses of DPT or DTaP is required. (DT requires a valid medical
contraindication).
Poliovirus Vaccine
A minimum of three doses of polio vaccine is required.
MMR (Measles, Mumps, Rubella)
One dose of MMR administered on or after first birthday is required. Doses received before first birthday will not be accepted. Laboratory evidence of immunity shall also be accepted.
HIB (Haemophilus B Influenza)
One to Three doses required. One dose must be administered on or after the
first birthday.
Varicella (Chicken Pox)
One dose administered on or after first birthday or proof of disease immunity required. Proof would include a written statement of past history of varicella disease by parent or physician, or serologic evidence of immunity.
Pneumococcal conjugate vaccine (PCV) At least one dose required and must be administered on or after first birthday.
Influenza vaccine At least one dose required between 9/1/11 and 12/31/11 for children through 59 months of age.
Please present documentation of any other immunizations your child has received, specifically for Hepatitis A and B vaccines. This information is important to maintain an accurate and complete immunization record for your child.
NO CHILD WILL BE ENROLLED PROVISIONALLY TO SCHOOL WITHOUT DOCUMENTATION OF AT LEAST ONE DOSE OF DIPHTHERIA, TETANUS, PERTUSSIS, POLIO, MEASLES, MUMPS, RUBELLA, HAEMOPHILUS B, PNEUMOCOCCAL CONJUGATE, VARICELLA, AND INFLUENZA WHICH ARE AGE APPROPRIATE FOR THE CHILD. The child must also have an appointment and be in the process of receiving the remaining immunizations.
A pupil is not required to have immunizations which are medically contraindicated according to Chapter 14, New Jersey State Sanitary Code. Written medical exemption from physician required. Pupils may also be exempted upon receipt of signed, written objections that the proposed immunizations interfere with the free exercise of the pupil’s religious rights.
